TOEFL 550 (paper-based); 80, with subscores of Reading 19, Listening 17, Speaking 20, and Writing 21 (iBT Internet-based TOEFL),
IELTS
IELTS 6.5, with subscores of 6.0 for all four subscores,
GRE
Tests Required GRE General; minimum of 145 Quantitative and 155 Verbal (tests taken August 2011 or after) OR minimum combined verbal and quantitative score of 1000 (tests taken prior to August 2011).

Submit a 400-word max UIC-specific essay answering the questions below.
Why is the UIC OT program a good match for you and why are you a good match for UIC? Think about the kind of OT you want to be, and how UIC will provide a means to get there. Answer the question by explaining how the UIC OT program links with your career plans and your prior experiences, interests, values or background.
GPA
Minimum GPA of 3.0 (on a 4.0 scale) for the last 60 semester hours earned that lead to a either a bachelor’s or master’s degree
You’re also expected to have a 3.0 GPA for your prerequisite coursework. If you have either a 60 hour GPA or a prerequisite GPA below 3.0, you may be considered for admission if you have strengths in other areas. Students with a GPA below 2.75 aren’t typically admitted.
Fee
There is a non-refundable application fee of $70 for all applicants.
RL
Submit three letters of recommendation in support of your application. Personal references are not acceptable. Recommenders are typically faculty, academic advisors, teaching assistants, or work or volunteer supervisors.
Transcript
International students: Submit original transcripts from non U.S. institutions, transcript evaluations and translations to OTCAS and the UIC OT Department. Provide an original transcript in the native language AND a translated copy in English. Translations should be on a translator’s letterhead and should be literal (not an interpretation). UIC only accepts translations from ATA certified translators, court-appointed translators, or from the consulate.
